How much do Televisits cost?
We are currently accept United Healthcare, Aetna, Anthem, Medicare and Medicaid insurances. We plan to start accepting more insurance plans in the future. In most cases, you will need to pay deductible only, but please check with your specific insurance plan for details. Self pay appointments that are not done through insurance are at $75 per visit.
How does the Televisit work?
You will get a link prior to your appointment that you can open up. It works on Android, iPhone and on your computer. Once you and the physician both enter Televisit chat room, we can discuss your medical problems. For the initial appointment, we will have to gather your basic medical history. Please have your favorite pharmacy and lab name and address ready so we can add it to your profile.
What medical records system secure?
We use a HIPAA compliant electronic medical records system, which is HIPAA compliant. There's also a medical portal, which is able for the patients to view securely through a website.
Are you board certified?
Yes Dr. Sikorevich and Dr. Polyakova are both board certified in family medicine.
Can I get prescriptions sent to my pharmacy?
We can send prescriptions to your pharmacy in Indiana. The types of prescriptions we are willing to send is at the discretion of the provider. The provider may require bloodwork or medical records prior to sending the prescriptions to the pharmacy. Please make sure your name and date of birth are correct in the system since some pharmacy records do carry over from Indiana pharmacies into our system, helping us to verify your medications. We do not prescribe controlled medications electronically, including but not limited to Norco, Percocet, and Xanax.
Can you order bloodwork?
Yes, we can send bloodwork orders to your Indiana lab, and they will send us your results.
How do I get ready for my Televisit?
Please complete all “intake forms”, such as signing HIPAA form in your portal. There are total of 5 forms to sign prior to the appointment. We will send you the link so you can view and sign your forms electronically.
Can you order imaging?
We can write orders for x-rays, CT scans, MRIs, screening mammograms and bone density scans to your imaging center of choice at the provider's discretion.
How is bloodwork and imaging billed?
We advise you call your insurance ahead of time to find out which is the preferred lab and imaging centers for your insurance so we can send the orders to appropriate location. The lab and imaging will be billed directly to your insurance carrier by the lab or imaging provider where you decide to go.
Can you make a referral?
Yes, we can refer you to a specialist or physical therapy, counseling, etc. if needed.
Do you have a physical office location?
Not yet. We are currently in the process of setting up a physical office location, which will be located in Cedar Lake, IN.
